Every lockout/tagout procedure follows the same basic process. OSHA requires a written, standardized LOTO procedure for each machine — but all procedures include these 6 core steps.
Identify the equipment and all energy sources
Review the written lockout procedure
Notify affected employees
Gather the correct LOTO devices
Preparation prevents surprises.
Power down the machine using normal shutdown procedures
Press stop buttons, switches, dials, etc.
Make sure all movement has stopped.
Disconnect, block, or close all primary energy sources:
Electrical: breakers, plugs
Hydraulic: valves
Pneumatic: air shutoffs
Gravity: secure with pins, blocks, etc.
No energy = no hazard.
Apply lockout devices to all isolation points
Attach tagout devices with clear info (name, reason, date)
One lock per authorized worker — no shared locks or keys
You control your own lock.
Bleed off air, drain pressure, block moving parts, discharge capacitors
Check flywheels, springs, and elevated components
Verify zero-energy state
Hidden energy is still dangerous.
Test controls to ensure machine won’t start
Then return controls to “off” or “neutral” position
Never skip this step — it proves you’re safe to begin work.
